How to register

All patients are registered with the practice but can express a preference to see a particular doctor, this preference should be stated when registering.

As part of General Medical Service contract, from April 201 all practices are required to provide all patients including children wil a Named Accountable GP who will have overall responsibility for the care and support that the surgery provides to them. Having a Named Accountable GP does not prevent or restrict you from seeing any other GP or nurse in the Practice.

The practice has an open appointment system which allows all patients to consult any of the doctors.

To register as a patient

Before filling out a registration form, check you are in our catchment area.

Please contact either surgery in person, where you will be asked to complete a Registration Pack

Information we will need to register you:

  • Date of birth
  • Full forenames and family name
  • Previous doctor and surgery address
  • Your previous address
  • Your current address
  • Telephone Number

Patients are also required to provide two forms of identification. Please click the link below for a list of identification acceptable.

If you have a medical card please bring it with you when you register. If this is your first registration at a General Practice in the UK, please bring your passport.
It is very important that you notify the Practice of any change of name, address or telephone number as soon as possible.
If you change your name for any reason other than marriage we will need to see evidence of your new name.
