In accordance with the Access to Health Records Act 1990 individuals have the right to access health records held by a healthcare provider who has treated the individual. The Data Protection ACT (DPA 2018) gives individuals the right to ask for a copy of information an organisation holds about them, the right is commonly know as Data Subject Access Request.
How do I access my medical record?
If you are registered for SystemOnline you can see your detailed coded information. To do this you must first send a request via SystemOnline, when you log in go to Account – Manage Online Services – Request Detailed Coded Record
If you wish to apply for access to your full medical record please complete the Data Subject Access Request Form and either return it by hand, post or email to patientservices@nhs.net. Please address your request to Mrs Lynn Garrard.
