Children Under 14 years of age should ideally be accompanied to their appointment by their parent or legal guardian
However we do appreciate that this is not always possible and very often relatives or friends are asked to accompany the child on the parent’s behalf
In these situations we would ask that the attending adult has written consent with them form the child’s parent or legal guardian permitting them to act in loco-parentis
The written consent must contain the child’s name, the name of the attending adult and an immediate contact number for the parent or legal guardian.

To make it easier there is a pro-forma available to download and complete if that is preferable.
